Please review the liability waiver and assumption of risk agreement, then complete the form below before entering the facility.

Important Legal Notice

By signing this agreement, you are waiving certain legal rights, including the right to sue. This agreement applies to injury, illness, or death resulting from participation in activities at the facility, even if caused by negligence.

Liability Waiver and Assumption of Risk Agreement

By entering and using the indoor playground facilities operated by Funfinity Indoor Playground (the "Facility"), I, the undersigned parent/legal guardian, acknowledge and agree to the following:

  1. Assumption of Risk

    I understand that the Facility includes, but is not limited to, trampolines, spiral slides, and sand pits containing small bead-like materials. I acknowledge that these activities involve inherent risks, including but not limited to falls, collisions, choking hazards, serious injury, illness, or death. I voluntarily assume all such risks on behalf of the child(ren) under my care.

  2. Mandatory Supervision

    I understand and agree that all children must be accompanied by a parent, legal guardian, or responsible adult caregiver at all times. Children must remain under active and direct adult supervision while inside the Facility. Funfinity Indoor Playground does not provide child supervision.

  3. Choking Hazard Warning

    I acknowledge that certain play areas, including sand pits containing small bead-like materials, present a choking hazard that could result in serious injury or death. I accept full responsibility for ensuring that the child(ren) do not ingest or misuse these materials.

  4. Rules and Safe Conduct

    I agree that the child(ren) will follow all posted rules and any instructions provided by staff. Failure to comply may result in removal from the Facility without refund.

  5. Release of Liability

    To the fullest extent permitted by the laws of the Province of Ontario, I hereby release, waive, and discharge Funfinity Indoor Playground, its owners, employees, and affiliates from any and all liability for any injury, illness, damage, or death arising from the use of the Facility, even if caused by negligence.

  6. Indemnification

    I agree to indemnify and hold harmless Funfinity Indoor Playground from any claims, demands, actions, or proceedings, including those involving serious injury or death, brought by or on behalf of the child(ren) or any third party as a result of participation in activities at the Facility.

  7. Acknowledgment

    I confirm that I have read and fully understand this agreement. I am aware that by signing this waiver, I am giving up certain legal rights, including the right to sue. I sign this agreement freely and voluntarily and intend it to be legally binding.
